Order Procedures:
Complete this form and it will be e-mailed to KYPBL. Within 48 hours, you
will receive an email from Scott Martin, State Adviser, stating the total for
your order (including shipping) and the address to mail the check or money
order. At that time, you may complete the order by mailing your check or
money order to the address you receive via email. Please allow 2-3 weeks
for delivery AFTER receipt of payment.
